Job summary

Liberty Health is seeking a Regional Hospice Admissions Nurse (RN) in Southern Pines, NC. You will evaluate and admit patients, initiate individualized care plans, and coordinate home-based services with thorough documentation.

Expect on-call rotation and monthly staff meetings as part of the schedule. Requires NC RN license with at least one year of clinical nursing experience, strong communication, organization, and time management skills, and willingness to travel regionally.

Qualifications

• Active RN license in North Carolina.
• One year of clinical nursing experience.
• Graduated from an approved school of nursing.
• Ability to provide home-based patient care with thorough documentation of the initial visit.
• Willingness to travel regionally and work in patients’ homes.

Responsibilities

• Evaluate and admit patients per LCD guidelines provided by MAC.
• Initiate and develop an individualized plan of care for each patient.
• Coordinate patient care services as necessary.
• Document comprehensive assessment at point of care.
• Work schedule includes on-call rotation and monthly staff meetings.
