Job summary

Enhabit Home Health & Hospice is seeking an experienced Registered Nurse to provide and coordinate in-home patient care in the Temple, Texas area. You will assess needs, implement care plans, and collaborate with a multidisciplinary team to deliver high-quality home health services.

The ideal candidate holds an Texas RN license, has at least one year of home health or hospice experience, and is proficient with mobile devices and EHRs to document care and communicate with the care team.

Qualifications

• Must have a nursing degree and be licensed as an RN in Texas.
• Experience coordinating care across interdisciplinary teams is preferred.
• Ability to use mobile devices and EHR systems for bedside documentation.

Responsibilities

• Coordinate patient care and continually assess patient and family needs.
• Implement the interdisciplinary plan of care per physician orders.
• Ensure care delivery complies with federal and state regulations.

Responsibilities

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
