Job summary

Enhabit Home Health & Hospice is seeking a Registered Nurse to coordinate home health care, assess patient and family needs, and execute the interdisciplinary plan of care. You will perform services per physician orders and the established care plan, ensuring compliance with federal and state regulations.

Requirements include an approved nursing degree, an active state RN license, and at least one year of RN experience. A valid driver's license and reliable transportation are also required.

Qualifications

• Graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• At least one year experience as a licensed RN.
• Basic technological skills including operation of a mobile device.

Responsibilities

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
