Job summary

Enhabit Home Health & Hospice is hiring a registered nurse to coordinate care and continuously assess patient and family needs in the home health setting. You will implement the interdisciplinary plan of care under physician orders and ensure compliance with federal and state regulations.

The role requires an RN license in the state, at least one year of experience, and comfort with mobile devices. Medicare home health or hospice experience is preferred, with a driver’s license and reliable

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
