Job summary

Enhabit Home Health & Hospice is hiring an experienced registered nurse to coordinate comprehensive home-based care. You will assess patient and family needs, implement and monitor the interdisciplinary plan, and ensure services follow physician orders and regulations.

A valid RN license in the state and at least one year of RN experience are required. Candidates should be comfortable using mobile devices and basic tech tools, with Medicare/home health experience a plus.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

Education And Experience, Preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
