Job summary

Enhabit Home Health & Hospice is seeking a licensed RN to provide coordination of care in patients’ homes, assessing needs and implementing the interdisciplinary plan of care.

You will deliver services per physician orders, ensure regulatory compliance, and maintain qualified technology usage. A valid driver license and auto insurance are required; CPR certification is preferred. Enhabit is an equal opportunity employer.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient’s and family’s needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

Education And Experience, Preferred

• Medicare home health or hospice experience is preferred.

• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
