Job summary

Superior Hospice in the Twin Cities area is seeking a Hospice RN Float to support admissions, liaison work, and clinical coverage across multiple sites. You will review referrals, complete admissions, educate families, and coordinate transitions with hospitals and post-acute partners.

Travel throughout the 21-county service area is required, with on-call rotation and weekend coverage. This dynamic role offers independence, impact, and opportunities to grow within a locally led hospice

Qualifications

• Active Minnesota RN license is required.
• Hospice experience preferred or high-acuity background (ICU, oncology, palliative care, home health, ED).
• Experience with hospice admissions, care transitions, discharge planning, or referral management is highly preferred.

Responsibilities

• Review referral pipelines, intake updates, and clinical reports to identify hospice-appropriate patients.
• Partner with intake to prioritize referrals, coordinate evaluations, and ensure timely admissions.
• Complete hospice admissions, including eligibility assessments, patient/family education, informed consents, and start-of-care documentation.
• Conduct informational visits with hospitals, SNFs, senior living communities, physicians, and discharge planners to support care transitions.
• Provide routine, PRN, and urgent visit coverage to maintain continuity of patient care.
• Collaborate with physicians, NPs, social workers, chaplains, aides, and inter-disciplinary team members.
• Help strengthen referral relationships and support organizational growth.
• Maintain accurate EMR documentation and ensure compliance with Medicare COP and hospice regulations.
• Step into clinical coverage needs across the organization to ensure seamless, high-quality patient care.

What You'll Do

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
• Hospice experience preferred, or a strong background in ICU, oncology, palliative care, home health, emergency medicine, or other high-acuity settings.
• Experience with hospice admissions, care transitions, discharge planning, or referral management is highly preferred.
• Strong clinical judgment, assessment skills, and patient education abilities.
• Excellent communication and relationship-building skills.
• Comfortable working independently in a field-based environment.
• Highly organized, adaptable, and responsive to changing priorities.
