Job summary

Superior Hospice seeks a Hospice Resource RN to lead admissions, visits, and case management across our service area. You will coordinate patient education, eligibility, and initial care planning while collaborating with a wide interdisciplinary team to ensure high-quality, patient-centered care.

You will perform routine, PRN, and urgent visits, maintain EMR documentation, and ensure compliance with Medicare requirements. A Minnesota RN license and willingness to travel are required.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred or strong background in ICU/oncology/palliative/h home health or other complex care settings.
• Excellent assessment skills and admissions experience.
• Strong communication and relationship-building with patients, families and interdisciplinary teams.
• Highly organized with ability to adapt in a fast-paced, field-based environment.
• Reliable transportation and willingness to travel throughout the service area.
• Compassionate, patient-centered approach to care.

Responsibilities

• Conduct hospice admissions, eligibility evaluations, patient education, informed consents, and initial care planning.
• Provide case management support, including symptom management, care coordination, and clinical oversight as needed.
• Complete routine, PRN, and urgent patient visits to ensure continuity of care.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide holistic, patient-centered care.
• Educate patients and families throughout the hospice journey with compassion and confidence.
• Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
