Job summary

Superior Hospice is hiring a Hospice RN Float to support admissions, case management, routine visits and urgent clinical needs across our growing program. The role calls for a licensed Minnesota RN with hospice or equivalent clinical experience and a willingness to travel within the service area.

You will collaborate with a multidisciplinary team to plan care, educate families, and document care in EMR while meeting Medicare conditions. This position offers a competitive salary and benefits.

Qualifications

  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred or strong clinical background in ICU, oncology, palliative care, home health.
  • Excellent assessment skills, critical thinking, and independent clinical judgment.
  • Comfortable managing changing priorities while maintaining patient care.
  • Outstanding communication and relationship-building with patients, families, and teams.
  • Reliable transportation and willingness to travel in the service area.
  • A compassionate nurse who enjoys learning and problem-solving.

Responsibilities

  • Complete hospice admissions, eligibility evaluations, informed consents and education.
  • Provide case management support including symptom management and care coordination.
  • Complete routine, PRN, urgent, and crisis field visits across Twin Cities metro area.
  • Collaborate with physicians, NPs, social workers and team to deliver interdisciplinary care.
  • Educate patients and families on disease progression and hospice services.
  • Maintain timely EMR documentation ensuring Medicare and hospice compliance.
  • Adapt quickly to changing clinical priorities while delivering compassionate care.

Schedule: Monday-Friday with On-Call and Weekend Rotation

What You'll Do

  • Complete hospice admissions, eligibility evaluations, informed consents, education, and initial care planning.
  • Provide case management support as needed, including symptom management, medication oversight, and ongoing care coordination.
  • Complete routine, PRN, urgent, and crisis field visits throughout the Twin Cities metro area.
  • Collaborate closely with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide exceptional interdisciplinary care.
