Job summary

Superior Hospice in Minnesota is seeking a Hospice RN Float to support admissions, case management, and field visits across the Twin Cities metro area. You’ll be the flexible, trusted clinician who helps patients and families navigate hospice with expert clinical care.

In this role you’ll complete admissions and care planning, coordinate with the care team, and provide symptom management and urgent support as needed. A compassionate, organized nurse will thrive here.

Qualifications

  • Active Minnesota RN license is required.
  • Hospice experience preferred or strong clinical background in related fields.
  • Excellent assessment, critical thinking, and independent clinical judgment.
  • Willingness to travel throughout the service area.

Responsibilities

  • Complete hospice admissions, eligibility evaluations, informed consents, education, and initial care planning.
  • Provide case management support including symptom management and care coordination.
  • Complete routine, PRN, urgent, and crisis field visits across the Twin Cities metro area.
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers for interdisciplinary care.
  • Educate patients and families on disease progression, comfort-focused care, and hospice services.
  • Maintain timely EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
  • Adapt quickly to changing clinical priorities while delivering compassionate, patient-centered care.

Company: Superior Hospice
Compensation: $90,000-$130,000 annually, depending on experience
Schedule: Monday-Friday with On-Call and Weekend Rotation

What You'll Do

  • Complete routine, PRN, urgent, and crisis field visits throughout the Twin Cities metro area.

  • Educate patients and families on disease progression, comfort-focused care, and hospice services.

What We're Looking For
  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or strong clinical experience in ICU, oncology, palliative care, home health, emergency medicine, or other complex care settings.
  • Excellent assessment skills, critical thinking, and independent clinical judgment.
  • Comfortable managing changing priorities while maintaining exceptional patient care.
  • Outstanding communication and relationship-building skills with patients, families, and interdisciplinary teams.
  • Reliable transportation and willingness to travel throughout our service area.
  • A compassionate nurse who enjoys learning, problem-solving, and making a difference wherever needed.
