Job summary

Capital City Hospice in Columbus, OH is seeking an Registered Nurse (RN) for Hospice On Call / Triage. The role involves triaging after-hours calls, coordinating care with the interdisciplinary team, and providing compassionate end-of-life support to patients and families.

This position requires an accredited RN degree, an active Ohio RN license, CPR certification, and at least two years of nursing experience. A valid driver’s license and insurance are required.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice as a Registered Nurse in the state.
• Minimum of two years of varied professional nursing experience.
• Demonstrated nursing practices and clinical judgment.
• Ability to cope with family emotional stress and different lifestyles.
• Must possess current CPR Certification; valid driver’s license and insurance.

Responsibilities

• Triage after-hours patient and family calls; determine services needed and delegate visits.
• Gather data on physical, psychological, social and spiritual factors and initiate interventions.
• Coordinate care with attending physician and interdisciplinary team.
• Perform clinical pronouncement of death and support families.
• Maintain accurate clinical records and care plans.

Location: Columbus, OH and surrounding areas

Schedule: Triage every other Sat/Sun 8a-8p and 2 12hr extender shifts/week

What You’ll Do

• Assume responsibility for triage of after-hours patient and family phone calls, determining the need for services including assessments, symptom management and care plan updates, delegating nursing visits to appropriate staff members
• Obtain data on physical, psychological, social, and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team as appropriate
• Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family
• Maintain an accurate clinical record through the timely preparation of care plans and clinical and progress notes

Qualifications

• Graduate from an accredited registered nursing program
• Possession of a current license to practice in the state as a Registered Nurse
• Minimum of two years of varied work experience as a professional nurse
• Demonstrated skill in nursing practices and clinical judgments
• Ability to cope with family emotional stress and tolerance of individual lifestyles
• Must possess current CPR Certification
• Valid driver’s license and proof of insurance is required
