Job summary
At Home Quality Care seeks an experienced hospice RN to plan, organize and direct patient care in home and community settings across Illinois.
The RN conducts comprehensive assessments, develops and revises care plans, administers prescribed therapies and medications, and educates patients and families.
Collaborating with an interdisciplinary team, you provide compassionate end‑of‑life care, ensure safety, and support families through challenging times.
Qualifications
- Graduate of an accredited nursing program with active state license.
- 1–2 years acute care experience and 1–2 years home hospice experience preferred.
- CPR certification and excellent communication skills.
- Ability to lift and assist patients; valid driver's license.
Responsibilities
- Completes initial and ongoing assessments of patients and families to determine hospice needs.
- Provides nursing care using the nursing process and plans of care.
- Develops and revises care plans based on patient status; communicates changes to team.
- Administers medications and treatments as prescribed by physicians.
- Educates patients and families; coordinates with interdisciplinary team.
