JourneyCare Hospice in Chicago, IL seeks Hospice RNs for an immediate opening supporting patients and families in home and facility settings. You will assess needs, coordinate with the interdisciplinary team, and provide compassionate end-of-life care in a respectful environment.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice as an RN in Illinois.
• Minimum of two years of professional nursing experience.
• Current CPR certification and ability to cope with emotional family stress.

Responsibilities

• Assess physical, psychological, social, and spiritual factors impacting patient and family needs.
• Coordinate care with attending physician and interdisciplinary team.
• Perform clinical death pronouncement and support to family.
• Maintain accurate care plans and progress notes.
