Job summary

Hospice of Grace and Palliative Care, Inc. in California is seeking a Registered Nurse to join our team. You will create and manage individualized care plans, monitor patients, and communicate effectively to support comfort at the end of life.

The ideal candidate holds a BSN, an RN license, and is skilled in collaborative care, disease management, and providing emotional support to families. This role requires compassionate, detail-oriented professionals comfortable with rotating shifts.

Qualifications

• BSN required.
• State-approved RN license.
• Hospice and palliative care certification preferred.
• Minimum of 2 years experience as a hospice nurse.
• Competency in electronic patient and hospice management systems (Hospice MD).
• Knowledge of monitoring and managing terminally ill patients.
• Strong analytical and diagnostic skills.
• Excellent interpersonal and communication abilities with compassion.
• Ability to handle stress and emotional challenges.
• Willingness to work rotating shifts.

Responsibilities

• Collaborate with physicians, social workers, chaplains, nursing assistants and other stakeholders in care of terminally ill patients.
• Prepare and adjust patient care plans as needed.
• Visit patients frequently to monitor vital signs and comfort.
• Administer prescribed treatments and pain management medications.
• Inform physicians about changes in patient conditions.
• Provide families with care information, end-of-life education, and emotional support.
• Maintain and update medical charts and health records.
• Notify attending physicians and family members of patient deaths.
• Advise grieving family members on coping techniques and available resources.
