Job summary

Hospice of Grace in Lomita, CA is seeking a Registered Nurse to create and manage patient care plans with compassion and attention to detail. You will observe and communicate in the patient’s best interest, delivering personalized end-of-life care.

You will collaborate with physicians, social workers, chaplains, and staff; monitor vitals, manage pain, educate families, and maintain accurate health records. Rotating shifts offer diverse experience.

Qualifications

• BSN required and active RN license.
• Minimum 2 years hospice nursing experience.
• Experience with Hospice MD or similar electronic records.
• Strong interpersonal and communication skills.
• Ability to manage stress and rotate shifts.

Responsibilities

• Collaborate with physicians, social workers, chaplains, nursing assistants, and others to coordinate care.
• Prepare and adjust patient care plans as needed.
• Monitor vital signs and comfort; administer pain management medications.
• Inform physicians of changes in patient conditions.
• Provide family education and emotional support.
• Maintain medical charts and health records.
• Notify team of patient deaths and offer resources.
• Assist grieving families with coping techniques and resources.
