Job summary

Affirmed Home Care, Hackensack, NJ, seeks an experienced Registered Nurse (RN) to provide personalized, one-on-one care to a client in the home in Hackensack, New Jersey.

This per diem, private-duty RN role offers day, evening, overnight and weekend assignments where available, with competitive pay. Join a supportive clinical and administrative team focused on compassionate, quality home care.

Qualifications

• Active New Jersey RN license.
• Minimum of 1 year of RN experience in acute care or home care.
• Minimum of 1 year of hospice, palliative, or end-of-life care experience.
• Current BLS certification.
• Valid driver’s license and reliable transportation.
• Authorized to work in the United States.
• Physical examination within the past year.
• PPD/QuantiFERON within past year or chest X-ray within 5 years, if applicable.
• Proof of flu vaccination or approved medical exemption.

Responsibilities

• Provide one-on-one private-duty home care.
• Manage patient pain and symptom relief in home setting.
• Document care activities and communicate with family and clinical team.
• Adhere to end-of-life care best practices.
