Job summary

Hackensack Meridian Health At Home seeks compassionate nurses to provide in-home patient care with a focus on one-on-one visits. The role emphasizes flexible scheduling and meaningful patient interactions in patients’ homes.

Experience in home health or hospice is preferred; RN or LPN licensure is required. The organization supports professional growth and a comprehensive benefits package in a community-based setting.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC Experience/Skills.

Responsibilities

• Provide professional nursing care to patients at home, assess needs, develop and evaluate care plans.
• Ensure physician orders are followed; documentation reflects care progression.
• Document clinical decisions, ensure safety and meet standards of care.
• Coordinate with payors and obtain necessary authorizations for services.
• Update care plans within two weeks of status changes.
• Instruct patients and families with teaching strategies aligned to the plan of care.
• Communicate with the health care team and coordinate referrals as needed.
• Supervise home health aides and evaluate their performance.
• Stay current with new standards and incorporate them into practice.
