CommuniCare Health Services in Independence, OH is seeking a Hospice RN Field Nurse to plan, organize, and direct home care services with an emphasis on community health education. You will build from community resources to meet patient needs in their homes and communities.

The role requires active RN licensure and 1–2 years of acute care experience; a Bachelor's with home health care exposure is preferred. Valid driver's license and reliable vehicle are required.

Competenze

• Graduate of an accredited school of nursing.
• Active RN licensure.
• 1–2 years of recent acute care experience in an institutional setting.
• Bachelor’s degree with 1 year of home health care experience preferred.
• Must be a licensed driver with a vehicle in good working order.

Mansioni

• Complete initial patient and family assessment to determine home care needs.
• Supervise and direct LPN and Home Health Aide in care plan implementation.
• Develop patient education plans and direct LPN in executing them.
• Create and modify Home Health Aide care plans with patient feedback.
• Evaluate and revise plan of care as patient needs change.
• Use health assessment data to determine nursing diagnoses.
• Develop goals-based care plans incorporating therapeutic actions.
• Administer medications and treatments as prescribed.
• Identify discharge planning needs and prepare prior to discharge.
• Act as Case Manager for assigned caseload as directed by supervisor.
• Prepare clinical notes and communicate patient needs to physicians.
