Job summary

Dignity Care Hospice is seeking an on-call RN Field Nurse to support the on-call hospice team in southeast Ohio. The role involves ensuring a safe, comfortable environment for patients at home and collaborating with families to plan and monitor care.

Responsibilities include initial patient assessments, developing care plans, administering prescribed medications, educating families, and participating in on-call rotations.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license.
• Bachelor’s degree with one year hospice/home health experience preferred.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Provide a complete physical assessment and history of current and previous illness.
• Develop a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions.
• Re-evaluate and revise patient care needs as needed.
• Administer medications and treatments safely and as prescribed by the physician.
• Counsel and educate patients and families in meeting nursing and related needs.
• Provide health care instructions to the patient as appropriate per assessment and plan of care.
• Participate in on-calls.

Education

Graduate of an accredited school of nursing

Active RN license

Bachelor’s degree with hospice/home health experience preferred
