Qualifications

• Current, unencumbered RN license required.
• Hospice/home health liaison, admissions, healthcare sales or referral development experience strongly preferred.
• Minimum one year of healthcare sales, marketing or liaison experience; two+ years preferred.
• Established Boston/South Shore hospital, facility and referral-source contacts preferred.

Responsibilities

• Develop, expand and maintain strong referral-source relationships across the assigned market.
• Represent hospice services professionally with referral partners, patients, families and community partners.
• Educate referral sources, patients and families on hospice care, eligibility, services and transition planning.
• Identify new referral opportunities and strengthen existing accounts through field activity.
• Collaborate with internal clinical and operations teams to support referrals and smooth transitions into care.
