VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered care in field-based settings. You will manage symptom control, develop individualized plans, and collaborate with physicians, social workers, and chaplains to ensure comfort and dignity for patients at the end of life.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.

Responsibilities

• Lead patient-centered hospice care and develop comfort-focused plans.
• Serve as care coordinator across the interdisciplinary team.
• Support patients and families with education and empathy.
• Drive quality and compliance through accurate documentation.
