VitalCaring Group in Oklahoma is seeking a Hospice Registered Nurse (RN) to lead patient-centered end-of-life care in field-based settings.

You will coordinate plans of care, manage symptoms, and support patients and families with empathy, clinical excellence, and clear communication.

This role emphasizes autonomy within a collaborative interdisciplinary team, delivering comfort and dignity in homes, facilities, and community settings.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: assess patients and families, develop and manage comfort-focused care plans.
• Serve as the Care Coordinator: coordinate care with the interdisciplinary team.
• Support Patients & Families: provide education and emotional support during the journey.
• Drive Quality & Compliance: maintain documentation and participate in care planning.
