Job summary

Reliance Hospice and Palliative Care in Torrance, CA is seeking a full-time Hospice Registered Nurse (RN) to perform home visits and coordinate patient care in the Torrance area and surrounding communities. The RN will assess patients, develop and implement care plans, educate families, and collaborate with an interdisciplinary team.

CA RN license, CPR, and hospice/home care experience required; valid driver's license and reliable vehicle essential.

Qualifications

• Current CA RN license to practice professional nursing.
• Graduate of NLN-accredited nursing school.
• Current CPR certification.
• Driver's license and reliable automobile with CA insurance.
• Minimum of two years' hospice or home care experience preferred.
• Self-directed with ability to work with minimal supervision.

Responsibilities

• Performs initial and ongoing patient and family assessments to determine hospice needs.
• Delivers professional nursing care using the nursing process.
• Develops and updates care plans in collaboration with the interdisciplinary team.
• Educates patients and families on self-care, medications, and disease management.
• Acts as case manager and coordinates patient care for assigned caseload.
• Document and communicate patient status and changes to the physician.
• Participates in on-call duties per policy and ensures necessary equipment and services are available.
