Reliance Hospice and Palliative Care seeks a compassionate Full-Time LVN for field-based palliative visits in the Torrance area. You will provide direct nursing care, administer medications, educate families, and coordinate interdisciplinary visits with RN supervision.

Qualifications

• Active California LVN license (required).
• Minimum 2 years of hospice or home health experience (required).
• Current CPR certification (required).
• Valid California driver's license and proof of auto insurance.
• Strong communication, documentation, and organizational skills.
• Ability to work independently in home and community settings.
• Phlebotomy and/or IV certification (preferred).
• Spanish speaking candidates (preferred).

Responsibilities

• Provide direct nursing care to hospice patients in private homes, assisted living facilities, board and care homes, and skilled nursing facilities under RN supervision.
• Perform and document nursing interventions in accordance with the Plan of Care.
• Assist with patient assessments, orders, and coordination of interdisciplinary visits.
• Administer medications and treatments as ordered.
• Monitor patient condition and report changes to the RN.
• Educate patients and families on care needs and symptom management.
• Triage calls as needed and attend death visits.
• Maintain accurate, timely documentation.
• Preserve patient dignity and comfort at end of life.
• Make scheduled, requested, and Continuous Care visits as required.
