Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead compassionate, patient-centered end-of-life care in patient homes and facilities. This field-based role emphasizes presence, empathy, and clinical excellence, partnering with families to honor goals and comfort.

You will coordinate care with an interdisciplinary team, manage symptoms, and advocate for patients and families through each visit with autonomy and support from a dedicated team.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care with comprehensive assessments.
• Develop and manage individualized, comfort-focused care plans.
• Deliver skilled nursing care with focus on pain and symptom management.
• Act as case manager coordinating care across interdisciplinary team.
• Collaborate with physicians, social workers, chaplains, and aides.
• Monitor patient condition and adjust care plans to maintain comfort.
• Provide education and emotional support to patients and families.
