Job summary

VitalCaring is seeking a Field-Based Hospice Registered Nurse (RN) to lead patient-centered care with dignity and compassion in home and facility settings. You will manage plans of care, assess patient needs, and champion comfort-focused outcomes while coordinating with physicians, social workers, and the care team.

The ideal candidate holds an active RN license, at least 1 year of nursing experience, strong clinical judgment, and excellent communication.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead hospice patient care and plan of care with focus on comfort
• Manage symptoms and coordinate with interdisciplinary team
• Provide education and emotional support to patients and families
• Document accurately and maintain quality/compliance
• Act as care coordinator across physicians, social workers, chaplains, and aides
