Three Oaks Hospice is seeking an experienced Registered Nurse to provide skilled nursing care at patients’ homes and coordinate care with the interdisciplinary team. This role emphasizes patient-family support, end-of-life care planning, and on-call coverage across Cartersville/Calhoun areas.

Interviews are held on-site in Woodstock. A valid RN license, CPR, and driver’s license are required, along with strong communication and organizational skills to ensure high-quality, compassionate care.

Qualificações

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as a RN and a valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal & communication skills; ability to support patient and family.

Responsabilidades

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team and patient/family.
• Perform comprehensive assessments of patients’ physical, psychosocial, and environmental needs.
• Implement the POC and revise the plan as needed.
• Educate patients/families about disease process, self-care, end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing progress toward goals.
• Coordinate all patient/family services with IDT members.
• Inform physician and staff of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability to meet patient needs on admission and during care.
• Communicate information using current organizational processes and systems.
• Apply admission/re-certification criteria for hospice care.
• Perform on-call duties for patients/families as assigned.
• Other duties as assigned.
