Job summary

Three Oaks Hospice in Forsyth and Dawson Counties is seeking a compassionate Registered Nurse to administer skilled nursing care in patients’ homes or residence. You will coordinate care with the interdisciplinary team and ensure high-quality end-of-life support.

The role requires an Associate or Baccalaureate in Nursing, an active RN license, CPR certification, and at least two years of RN experience (hospice/home health preferred). On-call duties may be required.

Qualifications

• RN licensed in state with active license
• Two years’ RN bedside experience; hospice/home health preferred
• Current CPR certification
• Proficient with computers and basic office software
• Strong communication and organizational skills, ethical standards

Responsibilities

• Perform comprehensive nursing assessments and coordinate care with the interdisciplinary team
• Develop and modify the plan of care with updates as needed
• Educate patients and families about disease process, end-of-life care, and self-care techniques
• Provide on-call coverage as assigned and respond to patient needs
• Maintain thorough documentation and progress notes
• Coordinate services with referrals and other providers
