Three Oaks Hospice is seeking a compassionate RN to provide skilled nursing care to patients in their homes within Snellville, Grayson, and Loganville areas. You will coordinate with the interdisciplinary team, support patients and families, and manage care plans.

Applicants must have an accredited nursing degree (Associate or Bachelor’s), an active RN license, CPR, and at least two years of clinical RN experience; hospice/home health experience preferred.

Qualificações

• Graduate of an accredited nursing program with an active RN license.
• Two years’ RN experience in a clinical setting; hospice/home health preferred.
• Current CPR certification; computer proficiency required.
• Strong interpersonal and communication skills; ethical standards expected.

Responsabilidades

• Administer skilled nursing care in patients' homes across the service area.
• Coordinate care with the interdisciplinary team and family; manage care plans.
• Educate patient/family on disease, self-care, and end-of-life care; train staff as needed.
• Initiate preventive and rehabilitative nursing procedures; document progress notes.
• Coordinate services and prioritize needs with IDT; refer to other services as needed.
• Inform physicians of changes and evaluate patient response; adjust care scope.
• Assess caregiver capability and ensure appropriate support throughout care.
• Perform on-call duties and provide on-call service as assigned.
