Job summary

Three Oaks Hospice in Forsyth and Dawson Counties is seeking a dedicated Registered Nurse to deliver compassionate, home-based hospice care. You will coordinate with the interdisciplinary team, assess patient and family needs, and ensure adherence to the care plan while providing education about end-of-life care.

The role emphasizes clinical excellence, communication with families, and on-call coverage to support patients throughout the care journey.

Qualifications

• Current RN license
• Two years RN experience in clinical care; hospice/home health preferred
• CPR certification is required

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients' needs at home and coordinate with the IDT
• Implement and update the plan of care as needed
• Educate patients and families about disease, end-of-life care, nutrition, and self-care
• Provide on-going nursing care and appropriate preventive and rehabilitative procedures
• Prepare clinical notes and progress reports documenting goals and outcomes
• Coordinate patient services and communicate changes with physicians and team
• Participate in on-call duties as assigned
