Job summary

Amedisys Home Health is seeking an experienced RN for full-time daytime shifts in Alabama. You will perform initial assessments, develop and revise care plans, and collaborate with the care team to maintain continuity of patient care at home.

Qualifications

• Current RN license for the state you work in.
• One year of nursing experience preferred; new grads may qualify for RN program.
• Current CPR certification and valid driver's license.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the total plan of care with the care team.
• Regularly communicates patient progress to the clinical manager and physician.
• Develops and maintains individualized patient care records.
• Notes pain/symptom changes and outcomes as appropriate.
• Educates families on patient condition and needs.
