Job summary

Bristol Hospice seeks a compassionate RN to join our Madison team. You will perform comprehensive patient assessments, deliver skilled nursing care, and collaborate with an interdisciplinary team to meet patient and family needs.

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of home care experience preferred.
• Must have a current nursing licensure in the state.
• Able to travel throughout the Bristol Hospice Service Area.
• Understand the hospice philosophy and care needs of terminally ill patients.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Deliver professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Coordinate with the interdisciplinary team to meet patient needs.
• Document pain and symptom management and communicate with physicians as required.
• Include patient and family in planning and teaching self-care techniques.
• Travel across Bristol Hospice service area as needed.
