Job summary

Bristol Hospice in Wisconsin is seeking a compassionate Registered Nurse to join our patient- and family-centered hospice team. You will perform comprehensive assessments, develop care plans, and manage pain and symptoms using the nursing process.

You will communicate with physicians, educate patients and families, collaborate with the interdisciplinary team, and travel within the service area as needed. A Wisconsin RN license and acute care experience are required.

Qualifications

• Requires at least 1 year of acute care experience in an institutional setting.
• 1 year of home care experience is preferred.
• Current licensure as an RN in the state of Wisconsin.
• Willingness to travel within the Bristol Hospice service area.
• Flexibility in work hours and shifts.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate patient’s status by writing and initiating plan of care; regularly re-evaluate and revise as needed.
• Initiate and revise the plan of care as patient status changes; document outcomes.
• Communicate with the physician regarding patient needs and obtain orders as required.
• Teach the patient and family self-care techniques; collaborate with the interdisciplinary team.
• Record pain/symptom management and changes/outcomes as appropriate.
• Work in concert with the interdisciplinary group; perform other duties as assigned.
