Job summary

3HC Home Health and Hospice is hiring a Hybrid HH & Hospice Nurse, RN (PRN) in Goldsboro, NC. The role provides intermittent skilled nursing care and coordinates hospice services in accordance with agency policies and hospice philosophy.

Ideal candidates hold a North Carolina RN license, have at least 1 year of acute care nursing experience, and preferably experience with terminally ill patients. CPR certification, EMR proficiency, strong communication, and reliable transportation are required.

Qualifications

Registered Nurse licensed to practice in North Carolina.
Minimum of one year of professional nursing experience in an acute care setting.
Experience with terminally ill patients preferred.
Maintain current CPR (BCLS) certification.
Basic computer skills and EMR experience.
Strong communication with patients, families, staff, physicians, and the public.
Dependable, tactful, and able to handle sensitive situations.
Competent to perform delegated tasks to support patient outcomes.
Possess reliable transportation.

Responsibilities

Abides by Compliance Program and Code of Ethics.
Creates positive experiences for internal and external customers; demonstrates courtesy and knowledge of services.
Manages assigned cases and assists office with positive patient outcomes and budget; provides skilled nursing care per physician plan.
Completes OASIS and assesses patient/home needs to plan care.
Provides comfort and end-of-life care, coordinates hospice services, and participates in QAPI/QI projects.
