Job summary

Agape Hospice Care seeks a dedicated RN to administer skilled nursing care in patients' homes, coordinating with the interdisciplinary team and families. You will implement the plan of care, educate families, and provide compassionate end-of-life support with on-call responsibilities.

The role requires an RN license, CPR, and at least two years of clinical RN experience; hospice experience is preferred. This on-site position emphasizes empathetic communication and strong organization.

Qualifications

• RN with two years in a clinical setting, hospice/home health preferred.
• Graduate of an accredited nursing program (ASN/BSN).
• Current state RN license and valid driver’s license.
• CPR certification and computer proficiency expected.

Responsibilities

• Conduct initial and ongoing assessments of patients and family needs.
• Coordinate care with IDT and referrals; develop and revise care plans.
• Educate patients and families about disease, end-of-life care, and self-care.
• Initiate preventive and rehabilitative nursing procedures.
• Document progress and coordinate services across teams.
• Provide on-call nursing coverage as assigned.
