Job summary

Gentiva is seeking a Registered Nurse (RN) – Hospice for a staffing role in Fairview Heights, Illinois. You will deliver hands-on nursing care in patients’ homes or approved settings, guided by an individualized Plan of Care.

Qualifications

• Registered Nurse with active license in Illinois.
• Minimum 1 year RN experience in hospice, palliative care, home health, or related.
• Hospice experience preferred; CPR certification required.

Responsibilities

• Serve as primary contact for patients and families throughout hospice care.
• Deliver and document skilled nursing care per Plan of Care in home or facility.
• Perform initial and ongoing patient assessments; adjust care as needed.
• Collaborate with IDG (physicians, LPNs, CNAs, social workers, chaplains, etc.).
• Provide education, support, and bereavement guidance to patients and families.
• Educate caregivers on safe, confident home care practices.
