Job summary

ExcelaCare Hospice is seeking a full-time Licensed Practical Nurse to provide skilled nursing care in patients’ homes under the supervision of an RN. You will coordinate with the care team, families, and referring agencies to ensure quality end-of-life care.

Responsibilities include administering medications, documenting patient status, and participating in on-call rotations. Opportunities to contribute to compassionate, patient-centered care in a supportive team environment.

Qualifications

• Graduate of an accredited School of Nursing for LVN or LPN.
• Experience: One year of nursing experience.
• License/Certification: Current State LVN/LPN license; CPR; valid driver’s license and auto liabilityproof.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Coordinate care with multidisciplinary team and families.
• Administer medications and treatments per policy and guidelines.
• Document patient status and interventions timely and accurately.
• Participate in on-call rotation and home visits.
