Job summary

ExcelaCare Hospice in Amarillo seeks an experienced RN Case Manager to provide compassionate hospice care in patients' homes and facilities. You will coordinate care with the interdisciplinary team to ensure safety and quality, while supporting patients and families.

The role requires an RN license, CPR certification, and a commitment to dignity and respect in end-of-life care. Travel and on-call may be required.

Qualifications

• Graduate of an accredited school of nursing.
• Two (2) years of RN experience in a clinical setting; hospice preferred.
• Current valid RN license and CPR certification.
• Driver’s license and proof of auto liability insurance; no OIG exclusions.

Responsibilities

• Provide hospice nursing care in patients' homes or facilities.
• Develop and implement a written Plan of Care with physician orders.
• Collaborate with the interdisciplinary team and family caregivers.
• Document patient status accurately and in a timely manner.
• Educate patients and families on care plans and end-of-life options.
• Make visits as scheduled and participate in on-call rotation.
