Job summary

St. Croix Hospice seeks a Registered Nurse to provide day-to-day hospice patient care under the RN Case Manager, focusing on comfort and quality of life in home settings.

Qualifications

• Graduate of an accredited school of nursing is required.
• Associate degree in nursing is required.
• Must have a current RN license in state(s) of practice.

Responsibilities

• Performs all duties in accordance with the Nurse Practice Act and nursing principles.
• Completes initial and ongoing comprehensive assessment of patient and family.
• Develops and revises Plan of Care with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed and coordinated with physician.
• Provides education and instructions to patient and family/caregiver.
• Records accurate clinical notes and communicates changes to the physician.
