Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic care to patients and families in their homes. You will collaborate with an interdisciplinary team to create individualized care plans and guide end-of-life conversations with professionalism and empathy.

Requirements include an RN license and a nursing degree (ADN/BSN/MSN) with 1+ year of acute care experience; hospice/home health experience preferred. Competitive benefits are offered.

Qualifications

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of recent acute care experience required; hospice/home health experience preferred.
• Develop individualized care plans with the patient and family, updating as needs evolve.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with the patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.

• Rotate on-call duties and ensure seamless care transitions.
