Job summary

American Traveler is hiring a Primary RN for a Community Hospice position in Colorado. The role requires 2+ years of nursing experience and an active CO or Compact RN license.

You will provide compassionate, patient-centered hospice care in home and facility settings, coordinate with the interdisciplinary team, and support patients and families through education and symptom management. Office hours are Monday through Friday, 8am–5pm, with occasional weekend and on-call rotations.

Qualifications

• Active RN license (CO or Compact) is required.
• Minimum of 2 years of nursing experience.

Responsibilities

• Provide care in home, nursing homes, group homes, or assisted living facilities.
• Manage an assigned hospice caseload and coordinate care with the interdisciplinary team.
• Schedule: Monday–Friday, 8am–5pm.
• Occasional weekend and on-call rotation.
• Act as generalist across disease processes with symptom management expertise.
• Educate patients and caregivers to support goals of care and confidence.
• Lead and coordinate care with the interdisciplinary team for assigned patients.
• Deliver compassionate hospice care in a supportive environment.
