ARC Therapy Services, LLC seeks an RN with hospice leadership experience to plan, coordinate, and deliver high-quality interdisciplinary hospice care. The role ensures compliance with federal/state standards and clinical best practices across the care team.

The candidate will supervise nurses and clinicians, coordinate patient and family care, manage plans of care in the EMR, and engage in quality improvement while maintaining strong physician and caregiver communications.

Qualifications

• Minimum two years of experience in a licensed Medicare hospice agency.
• Competent with Federal Medicare and Local standards including Conditions of Participation and Local Coverage Determinations.
• Basic computer skills including Outlook, Word and Excel.

Responsabilités

• Supervise and assess performance of the hospice care team to ensure quality services.
• Coordinate patient and family care; lead interdisciplinary team meetings.
• Provide case management with accurate evaluation and treatment plans.
• Provide clinical oversight for hospice patients within the team.
• Review documentation to ensure Medicare and hospice criteria are met in medical records.
• Review and approve Plans of Care within the EMR system.
• Analyze team outcomes and participate in quality improvement initiatives.
• Ensure proper scheduling and coordination of patient care.
• Collaborate with physicians, referral sources, caregivers, and patients to maintain care continuity.
• Mentor hospice team on documentation and clinical standards; address deficiencies.
• Assist with orientation for hospice staff.
• Maintain communication with the Director of Clinical Services – Hospice regarding patient care and relations.
• Support quality assurance and improvement activities led by the DCS – Hospice.
• Provide performance feedback to hospice colleagues.
• Adhere to HIPAA and confidentiality standards.
• Support survey readiness and continuous readiness activities.
• Participate in on-call rotation for 24/7 patient care oversight.
