Optum is seeking a Hospice Clinical Manager to lead nursing and care coordination for home-based hospice services. The role oversees clinical performance, scheduling, referrals, and documentation to ensure high-quality, compliant patient care while maintaining fiscal responsibility.

The position includes management of staff and collaboration with the director of staffing to optimize care operations, with potential on-call and occasional direct patient care during emergencies.

Formación

• RN license current and in good standing for the state(s) assigned (CT).
• 1+ year of clinical RN experience in home health or hospice.
• 1+ year of RN management experience.
• Current CPR certification.
• Driver's License and access to reliable transportation.

Responsabilidades

• Supports, participates and works cooperatively with the director of staffing.
• Responsible for overall quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews requests for services and determines patient eligibility/suitability for home care services; coordinates patient referrals and admissions.
• Oversees scheduling and payor verification/precertification requirements.
• Reviews documentation to ensure completeness and timeliness.
• Assists clinicians in establishing Plan of Care (POC) and revisions as needed.
• Evaluates patient needs and updates the plan of care in collaboration with clinicians.
• May provide direct patient care in emergencies.
• Participates in the on-call process.
• Assists with staff training to ensure quality care and fiscal responsibility.
• Participates in investigating and resolving patient/family/physician concerns.
