Qualifications

• Active RN credentials with strong assessment, care planning and interventions.
• Experience leading teams in hospice, home health, acute care, or skilled nursing settings.
• Ability to communicate with empathy and professionalism with patients, families, clinicians.
• Competence in medication administration, IV therapy, clinical documentation.
• BLS certification and commitment to quality, safety, end-of-life care.

Responsibilities

• Oversee patient assessments, individualized care plans, clinical interventions, medication administration, and timely documentation.
• Collaborate with physicians, caregivers, social workers, and interdisciplinary team members to coordinate hospice and palliative care services.
• Use sound clinical judgment to address changing patient needs, manage complex symptoms, and ensure quality standards are met.
• Coach nurses and clinical team members through education, performance support, and best practices in skilled nursing and patient care.
• Monitor clinical operations, care outcomes, and compliance while supporting families.
