Job summary

Compassus is seeking a Hospice Clinical Liaison in Ohio to model our values and promote the Compassus philosophy. You will assist referral sources in identifying patients whose needs align with hospice services and facilitate admissions and transitions from hospital or long-term care settings to home hospice care.

The role requires an RN license in the relevant state(s) and 2–3 years of related experience, with hospital/long-term care experience highly preferred.

Qualifications

• RN license in state(s) of employment required.
• 2–3 years of related experience required.
• Hospital and/or long-term care clinical experience preferred.
• Healthcare marketing experience preferred.

Responsibilities

• Attends and participates in staff meetings, educational programs, and community events.
• Explains hospice services and benefits to patients, families, and significant others to foster understanding and acceptance.
• Assists referral sources in identifying patients whose needs match hospice services.
• Facilitates hospital and facility admissions and transitions to home hospice care.
• Coordinates patient discharge, transportation, DME, medication needs, and admission with a home hospice nurse.
• Provides education to hospitals and facilities on general inpatient care purpose and goals.
