Responsibilities

• Serve as a liaison between patients, families, and the Agency.
• Communicate with the Hospice Team regarding patient status.
• Conduct thorough patient assessments for transition planning.
• Coordinate referrals to services for patients and families.
• Ensure realistic and measurable goals on patient plans of care.
