Job summary

AdvisaCare Hospice is seeking a dedicated RN to join our Bay City/Midland/Saginaw service area as a Full-time Hospice RN Case Manager. You will provide compassionate, one-on-one home-based care focused on comfort and dignity, with families supported throughout the end-of-life journey.

The role requires an active Michigan RN license, at least 1 year hospice experience, and the ability to conduct field visits and on-call coverage. No on-call is required thanks to our dedicated after-hours team.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to adapt to patient status with full-time on-call basis.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Supervision of paraprofessionals providing services to patient.
• Participate in IDG conferences and Hospice performance improvement activities.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communication of information using current process and technology within organization.
