Job summary

JourneyCare Hospice in Illinois is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to oversee patient-centered, end-of-life care. The role features a Monday–Friday schedule with a single weekend rotation every 12 weeks, across Mundelein, Libertyville, and Grayslake.

Compensation ranges from $80,000 to $97,000 annually based on experience, plus benefits like 401(k), health insurance, PTO, mileage reimbursement, and tuition assistance.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.
