Job summary

JourneyCare Hospice in Chicago, IL is seeking a compassionate Hospice RN Case Manager to deliver patient-centered end-of-life care with a manageable caseload and strong interdisciplinary support.

As the primary RN Case Manager, you will conduct assessments, plan care, coordinate with physicians, and educate families while maintaining compliant documentation. This full-time role offers a Monday–Friday schedule, limited weekend calls, and comprehensive benefits.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.
