Job summary

JourneyCare Hospice is seeking a compassionate Hospice Registered Nurse (RN) Case Manager for a full-time role in the Chicago area. You will oversee a dedicated hospice caseload, perform assessments, and develop individualized care plans to ensure comfort and dignity for patients at end of life.

We offer a Monday–Friday schedule with a light weekend on-call rotation and a territory covering Hyde Park, Evergreen Park, Chicago, IL 60805, and nearby communities.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.
